Yep, negligence, but he was exhonerated, though he wasnt to popular with anyone for a while. Of course he just made things worse later:)
 
They court martialed him over the destruction of the Behemoth -- he was found not to have been at fault, though.

Much like Blair in WC2, though, he had trouble with his reputation afterwards despite a not-guilty verdict...
 
Could explain why he was so chummy with Blair at the beginning of WC4, he knows what it's like to have everyone suspicious of him. Of course, we know we have every reason to be suspicious of him in WC4! :)
